| 主旨： | 檢送「桃園市特殊教育學生情緒及行為問題支持資源中心115學年度第1學期電話諮詢服務實施計畫」及「桃園市高級中等以下學校特殊教育學生情緒及行為問題專業支持團隊三級個案服務實施計畫」各1份，請查照。 |
| 說明： |
| 一、 | 依據桃園市高級中等以下學校特殊教育學生情緒及行為問題 專業支援團隊實施計畫辦理。 |
| 二、 | 旨案計畫為透過電話諮詢服務，提供本市學校團隊或家長對特殊教育學生之情緒行為問題之了解與因應策略，以協助學生在校適應。 |
| 三、 | 本案諮詢資訊摘要如下： |
| (一) | 服務對象：本市各教育階段特殊教育學生之教師、家長及相關人員。 |
| (二) | 服務時間：即日起至116年1月15日，每週一、二、三、五上午8時至中午12時及每週四下午1時至4時。 |
| (三) | 服務專線：4560017。 |
| 四、 | 本案三級個案服務資訊摘要如下： |
| (一) | 服務對象與申請資格：桃園市公立高中職、國中、國小及學前教育階段之學生經本市特殊教育學生鑑定及就學輔導會鑑定為特殊教育學生 (含疑似生)，伴隨情緒行為問題者，並經學校介入輔導達一學期以上仍無成效者，或經教育局轉介之特殊個案。 |
| (二) | 服務申請方式： |
| １、 | 申請及轉介：由學校教師向情支中心提出轉介需求。 |
| ２、 | 申請程序： |
| (１) | 諮詢確認：先行致電情支中心(電話:03-4661231分機17)諮詢轉介程序 |
| (２) | 填表核章：至桃園市情支中心網站下載轉介表，填寫完成後進行校內核章。 |
| (３) | 檢附文件：個別化教育計畫（含行為功能介入方案）、半年內輔導紀錄、鑑定安置報告、個案會議紀錄、其他輔導介入資料或醫療資料等。 |
| (４) | 資料寄送：將上述資料郵寄送至情支中心個案輔導組辦理資料審查。 |
| 五、 | 旨揭實施計畫，亦可至「桃園市特殊教育資源網」→桃園市特殊教育科→表件下載區(網址：https://reurl.cc/L6poL4)下載運用。 |
