| 主旨： | 檢送本市115年度友善校園學生事務與輔導工作作業計畫－學生生命教育多元課程「3Q達人故事甄選活動－國小組」實施計畫1份，詳如說明，請查照。 |
| 說明： |
| 一、 | 依據本市115年度友善校園學生事務與輔導生命教育工作計畫辦理。 |
| 二、 | 旨揭計畫係引導學生分享生活經驗，發展生命潛能，建立敘說生命故事之能力，營造熱愛生命的友善校園。 |
| 三、 | 實施方式說明如下： |
| (一) | 活動對象：本市公私立國民小學中高年級學生，分AQ、EQ、MQ等三類3Q達人故事徵選。凡能展現3Q中之一特質，足為表彰之學生，皆可推薦參加，每一受推薦學生僅限參選一類；曾獲獎者不重複參加同類別，以發掘更多之表率學生。 |
| (二) | 報名資料送件 |
| １、 | 書面紙本資料：含報名表(正本-請簽章完整)、優良事蹟表及相關佐證資料(影本)。請將上述三項資料依序合併裝訂，於收件日期至115年10月23日前(郵戳為憑)寄送至忠貞國小輔導室輔導組收。忠貞國小地址：32468桃園市平鎮區龍南路315號；洽詢電話：(03)4501450轉615。 |
| ２、 | 電子檔：含報名表(word檔)、優良事蹟表(word檔)及學生照片檔(jpg檔)，請以e-mail寄至忠貞國小電子信箱：counseling@m5.jjes.tyc.edu.tw。信件主旨請填「○○國小3Q達人故事甄選送件」。若檔案太大而無法傳送，請電聯忠貞國小輔導組。 |
