| 說明： |
| 一、 | 依據本局115年5月21日桃教小字第1150046331號函及桃園市 立迴龍國民中小學（以下簡稱迴龍國中小學）115年6月15日 迴中小學字第1150005121號函辦理。 |
| 二、 | 旨揭計畫摘述如下（詳如實施計畫）： |
| (一) | 評選內容： |
| １、 | 紀錄片組： |
| (１) | 以「心三美運動」主題－問安、感謝、做好事，擇至 少一項為影片主要內容，進行活動中成員之側拍及訪 談記錄。 |
| (２) | 影片內以相片及影片混合剪輯方式呈現。 |
| ２、 | 創意組： |
| (１) | 以「心三美運動」主題一問安、感謝、做好事，擇至 少一項為影片主要内容，進行原創影片拍攝。 |
| (２) | 影片內容可以短劇、廣告、相聲、歌曲MV等多元方 式呈現。 |
| (二) | 收件規範： |
| １、 | 影片長度（含片頭及片尾）在2分30秒至3分30秒之間。 影片格式以可支援上傳至YouTube的檔案格式為主（例如：.MOV、.MPEG4、.MP4、.AVI、.WMV等）。 |
| ２、 | 每件參選作品至多填報3位主要作者，需填寫「心三美 典範遴選活動報名表」、「著作權無償授權同意書」及 「參選作品授權切結書」。 |
| (三) | 收件方式： |
| １、 | 線上繳交：請至https://forms.gle/3LXhRLoYFdLJZxTt5網頁連結填寫繳件表單並上傳相關檔案。 |
| ２、 | 郵寄：（如實施計畫附件一、二、三）連同影片光碟寄 （送）至迴龍國中小學務處（住址：333桃園市龜山區 萬壽路一段168號），電話：（02）8209–6088分機310 、317。 |
| (四) | 收件日期：自即日起至116年1月22日（星期五）止（郵戳 為憑）。 |
| (五) | 錄取獎勵： |
| １、 | 作者為桃園市所屬高級中等以下學校教職員或教育局及 所屬機關人員，依「桃園市高級中等以下學校教育人員 研究著作給分審查要點」核予著作分數0.1分（錄取作品如由多人合作者，給分依作者人數平均之）。 |
| ２、 | 特優、優等之作品獎勵如下： |
| (１) | 作者為桃園市市立各级學校及幼兒園教職員，依「公 立高級中等以下學校校長成績考核辦法」、「公立高 級中等以下學校教師成績考核辦法」及「桃園市市立 各級學校及幼兒園教職員獎懲要點」等規定核予嘉獎 1次。 |
| (２) | 作者為學生或非屬桃園市市立各级學校及幼兒園教職 員者，由本局核發獎狀1紙。 |
| (３) | 本局核發特優每件作品禮券5,000元；優等每件作品禮券3,000元。 |
| ３、 | 佳作之作品，由本局核發每位作者獎狀1紙。 |
