| 主旨： | 轉知「桃園市115年度模範身心障礙者、模範身心障礙家庭照顧者及模範身心障礙服務人員遴選計畫」公告一案，請查照。 |
| 說明： |
| 一、 | 依據本府社會局115年6月26日桃社障字第1150058642號函辦理。 |
| 二、 | 本案預計表揚對象35名，分述如下： |
| (一) | 模範身心障礙者：設籍本市，領有身心障礙證明者，計表揚10人。 |
| (二) | 模範身心障礙家庭照顧者：設籍本市，親自長期照顧領有身心障礙證明者之家屬，計表揚10人。 |
| (三) | 模範身心障礙服務人員：推行身心障礙福利有功人員且實際於本市提供身心障礙者服務，計表揚15人。 |
| 三、 | 案內推薦資料請於115年7月27日（星期一）前，以免備文方式逕送本府社會局身心障礙福利科（郵戳為憑，逾期不予受理），並請務必將檔案以電郵方式傳送至10021183@mail.tycg.gov.tw，方為完成推薦。 |
| 四、 | 旨揭計畫請逕至本府社會局首頁>福利服務>身心障礙者福利>國際身心障礙者日下載。 |
| 五、 | 隨文檢附本府社會局來函及遴選計畫各1份，倘有疑義請逕洽本府社會局承辦人温臻婷（電話：03-3322101分機6307）。 |
