| 主旨： | 檢送本局與長庚大學台塑企業文物館共同辦理「第14屆（114學年度）品德教育實踐獎」實施計畫1份，請踴躍推薦符合資格之學生，請查照。 |
| 說明： |
| 一、 | 依據長庚大學115年4月17日長庚大字第1150040186號函辦理。 |
| 二、 | 旨揭計畫相關資訊如下（詳如實施計畫）： |
| (一) | 申請資格：本市各公私立高中、國中及小學學生。 |
| (二) | 遴選指標：本獎項以「勤勞樸實」為核心價值，不以智育成績為評量項目，甄選品德優良足堪學校楷模者。「勤勞樸實」之意涵與實踐範例詳如實施計畫。 |
| (三) | 獎勵： |
| １、 | 本案評選結果產生後，將於115年9月19日(星期六)辦理頒獎表揚活動。 |
| ２、 | 獲獎學生獎額： |
| (１) | 實踐獎：獎狀1紙及新臺幣3,000元禮券1份。 |
| 甲、 | 高中：由本局另案函請學校核予記功1次。 |
| 乙、 | 國中：由本局另案函請學校核予記功1次。 |
| 丙、 | 國小：由各校逕依權責從優獎勵，以為嘉勉。 |
| (２) | 優良獎：獎狀1紙及新臺幣1,000元禮券1份。 |
| 甲、 | 高中：由本局另案函請學校核予嘉獎2次。 |
| 乙、 | 國中：由本局另案函請學校核予嘉獎2次。 |
| 丙、 | 國小：由各校逕依權責從優獎勵，以為嘉勉。 |
| (３) | 入選獎：獲獎狀1紙。 |
| 甲、 | 高中：由本局另案函請學校核予嘉獎1次。 |
| 乙、 | 國中：由本局另案函請學校核予嘉獎1次。 |
| 丙、 | 國小：由各校逕依權責從優獎勵，以為嘉勉。 |
| ３、 | 指導老師獎額：依「公立高級中等以下學校校長成績考核辦法」、「公立高級中等以下學校教師成績考核辦法」及「桃園市市立各級學校及幼兒園教職員獎懲要點」等規定，本案薦送老師（指導老師）獎額，如下： |
| (１) | 榮獲實踐獎學生之指導老師：每名嘉獎1次及獎狀1紙。 |
| (２) | 榮獲優良獎學生之指導老師：每名績優狀2張及獎狀1紙。 |
| (３) | 榮獲入選獎學生之指導老師：每名獎狀1紙。 |
| (四) | 薦送作業時程： |
| １、 | 115年6月12日（星期五）前薦送符合資格之優良學生，每校至多5名。 |
| ２、 | 薦送作業以線上報名方式，請薦送老師於報名期限內進入網址https://forms.gle/vZQMtPXD1xgka4W3A，完成撰寫申請表單資料、掃描上傳學生佐證資料(PDF，1件為限)，下載簽名及校方核章掃描上傳長庚大學台塑企業文物館。 |
