| 說明： |
| 一、 | 依據本局114年9月16日桃教體字第1140090522號函辦理。 |
| 二、 | 旨揭活動說明如下： |
| (一) | 活動時間：115年3月2日(一)起至115年3月27日(五)止。 |
| (二) | 活動對象：本市公私立各國小三到六年級學生。 |
| (三) | 實施方式： |
| １、 | 進入活動網頁可參加作答有三種方式，如下： |
| (１) | 請至潛龍國小首頁→左方點選「全民健保含正確用藥有獎徵答」。 |
| (２) | 直接輸入網址：https://www.cles.tyc.edu.tw/ |
| (３) | 掃瞄活動QR code(詳如附件)。 |
| ２、 | 進入網址進行填答前，請詳閱說明，一個帳號只有一次答題機會，完全正確者才有機會進行抽獎；另115年3月2日之前或115年3月27日之後作答者不予計算。 |
| ３、 | 活動截止後，將於115年4月22日(星期三)前用電腦亂數抽出435名，贈送精美獎品1份。 |
