| 一、 | 依據國立臺灣師範大學113年3月14日師大數學中字第1131006732號函辦理。 |
| 二、 | 旨案相關資訊如下： |
| (一) | 研習目的：希望藉由有趣的數學活動，對學習數學準備不足之學生，奠立學生學習數學的興趣與前置經驗。 |
| (二) | 報名資格： |
| １、 | 模組設計者（曾設計奠基、奠基進教室、109素養包、21世紀素養包或微課程模組教案其中之一項）。 |
| ２、 | 教學影片教師（曾拍攝過中心教學影片者）。 |
| ３、 | 講師（曾為1到4期活動師講師、109素養包準講師或21世紀素養包準講師其中之一項）。 |
| ４、 | 實際參加過第五期活動師培訓並曾申請過教學實作者。 |
| (三) | 名額：分為國小中年級組、國小高年級、國中組三組，每組最多開二班，每班以28名為原則，並視報名情況調整。 |
| (四) | 研習時間：113年3月30日(星期六)及3月31日(星期日)上午 9時至下午4時20分 |
| (五) | 研習地點：高雄市勝利國小(813高雄市左營區南屏路1號)。 |
| (六) | 報名方式：請於113年3月21日(星期四)中午12時前至以下網址（https://forms.gle/E3a2NVGwEb55UNJd8）填寫報名表，逾期恕不受理。 |
| (七) | 費用：本次活動不收取任何費用，有提供午餐，其他保險費、住宿及交通費請自理。 |
| (八) | 證書及參與證明: |
| １、 | 符合上述資格的數學教師，參加國立臺灣師範大學數學教育中心舉辦的「數學活動師講師」培訓工作坊，並完成所有課程內容(需上滿該組別每一模組課程)取得認證合格者，頒發該組別「第五期數學活動師講師」證書。 |
| ２、 | 為確保培訓品質，若其中未上滿該期模組課程2/3時數者，可繼續旁聽研習課程，但不給予第五期數學活動師講師證書。 |
| ３、 | 取得「第五期數學活動師講師」資格者，方可擔任第五期活動師培訓講師，並可申請辦理「數學奠基進教室教學實作」。 |

| 三、 | 請學校本權責核予公假登記參加。 |
