| 主旨： | 有關本校辦理桃園市114學年度教育部國民及學前教育署戶外教育與海洋教育計畫-學校推展優質戶外教育路線『虎頭山與蟲與鳥對話趣』活動，歡迎各校踴躍報名參加，請查照。 |
| 說明： | 報名期間:即日起至114年10月9日(星期四)截止 |
| 一、 | 依據114年9月15日桃教小字第第11400869901號函辦理。 |
| 二、 | 活動期程：114年10月至115年1月 (10月21日、10月27日、11月11日、11月18日和1月10日)，共計五梯次。 |
| 三、 | 活動地點：虎頭山。 |
| 四、 | 活動對象：各梯次學生30人，教師1人為限。 |
| 五、 | 報名方式：填妥報名表後e-mail至 csps300@csps.tyc.edu.tw，並來電確認。電話號碼：3374883 分機 310，唐主任。 |
| 六、 | 報名期間:即日起至114年10月9日(星期四)截止，依報名順序優先錄取，額滿為止。本校將以電子公文通知錄取學校。 |
| 七、 | 本活動已申請專案經費，補助如下: |
| (一) | 講座鐘點費5,400元(計3 小時，含講師及助教共2 人,由承辦學校安排講師)。 |
| (二) | 交通費每校補助1車7,000元． |
| (三) | 保險費每生補助50元(不含教師，每校30生，計1,500 元) ． |
| (四) | 膳費每生補助120元(不含教師，每校30生，計3,600 元)． |
| (五) | 共計補助每校17,500元。 |
| 八、 | 另請於活動結束後二週內，每校補助款之支出原始憑證正本(黏貼憑證皆需各校校內核章完妥)，檢據送回青溪國小學務處彙整並辦理經費撥付。檢據內容如下： |
| (一) | 統一收據(依實際金額開立，註明學校帳戶戶名及統編) |
| (二) | 活動學員名冊暨簽到表(附件二) |
| (三) | 學習成果(附件三) |
| (四) | 活動學習花絮(附件四) |
| (五) | 滿意度調查表(附件五) |
| (六) | 收支結算表(附件六) |
| (七) | 支出憑證簿(附件七) |
| (八) | 活動支出之原始黏貼憑證正本（請務必自行影印副本留存，供貴校校內核銷。) |
| 九、 | 檢附活動計畫、報名表、學員名冊暨簽到表及成果表(如附件)。 |
