| 主旨： | 轉知「教育部115年度全國終身學習楷模選拔實施計畫」 |
| 說明： |
| 一、 | 依據教育部115年4月7日臺教社（一）字第1152400855號函辦理。 |
| 二、 | 旨揭選拔對象分為「學習者」及「教學者」組，請各機關學校及團體踴躍推薦報名參加，連同推薦書及相關佐證資料，於115年4月24日（星期五）前將以下資料送至本局終身學習科黃先生辦理（免膠裝，以郵戳為憑，請註明「115年度全國終身學習楷模選拔」）： |
| (一) | 資料檢核表（附表1）1份。 |
| (二) | 報名表（附表2）正本1份及影本10份（正本需推薦單位用印）。 |
| (三) | 優良事蹟表（附表3）10份。 |
| (四) | 文字圖像影片使用同意書（附表4）及個人資料蒐集聲明暨提供使用同意書（附表5）正本各1份。 |
| (五) | 附件文件：學習/教學活動之照片、獎狀、獎盃照片、證書、報導、年資證明等佐證資料10份，每份資料以A4紙張10頁為限。 |
| (六) | 照片及影片： |
| １、 | 照片檔請提供正面照片1張（不限直式或橫式）及學習照片4張（限橫式），照片畫質1440*1080以上，檔案格式jpg檔。 |
| ２、 | 影片檔長度以1至3分鐘為限，影片畫質1080p（HD）1920*1080以上，檔案格式avi、wmv、mpg、mov檔。 |
| (七) | 光碟或USB隨身碟：上述文件（含前述報名表、優良事蹟表、附件文件、照片及影片等）之電子檔。 |
| 三、 | 相關表件請至「教育部全球資訊網∕認識教育部∕教育部各單位∕終身教育司∕資料下載」下載。 |
