| 說明： |
| 一、 | 依據教育部113年3月19日臺教社（一）字第1132400799號函辦理。 |
| 二、 | 請各機關學校及團體踴躍推薦報名參加，連同推薦書及相關佐證資料，於113年4月19日(星期五)前將以下資料送至本局終身學習科謝瑩露小姐辦理，請免膠裝(以郵戳為憑，請註明「113年度全國終身學習楷模選拔」)： |
| (一) | 資料檢核表（附表1）正本2份。 |
| (二) | 選拔報名表（附表2）正本 2 份及影本8份。 |
| (三) | 選拔活動優良事蹟表（附表3）11 份。 |
| (四) | 文字圖像影片使用同意書（附表4）及個人資料蒐集聲明暨提供使用同意書（附表5）正本各1 份。 |
| (五) | 附件文件：佐證資料11份，每份資料以 A4 紙張 10 頁為限。 |
| (六) | 照片及影片： |
| １、 | 照片檔請提供正面照片1張及學習照片4張，照片畫質1440*1080以上，檔案格式jpg檔。 |
| ２、 | 影片檔長度以1至3分鐘為限，1920*1080以上畫質，檔案格式avi、wmv、mpg、mov之影片。 |
| (七) | 光碟：上述文件（含前述報名表、優良事蹟表、附件文件、照片及影片等）之電子檔，請燒錄成光碟1式3份。 |
| 三、 | 為獎勵績優講師，給予教學肯定，並鼓勵優質講師投入終身學習行列，旨揭選拔對象分為「學習者」及「教學者」組，詳參附件。 |
| 四、 | 相關表件請至「教育部全球資訊網∕認識教育部∕教育部各單位∕終身教育司∕資料下載」下載。 |
| 五、 | 教育部委託國立臺灣科學教育館辦理首揭計畫，如需諮詢，可洽計畫團隊（何依庭專任助理，02-6610-1234分機1526）。 |
