| 說明： |
| 一、 | 依據桃園市政府113年5月9日府社團字第1130126323號函辦理。 |
| 二、 | 旨揭頒授要點有關服務年資及時數之計算，係自90年1月22日(「志願服務法」公布施行)起算至113年5月31日止，並僅限於候選人在推薦單位之服務年資及時數，頒授對象及頒授標準請詳參頒授要點。 |
| 三、 | 旨揭獎項推薦日期，自113年6月1日起至6月30日止，以郵戳為憑，逾期或所送表件不全者概不受理；推薦表一律請以電腦WORD標楷體14字型、固定行高20pt，橫書繕打一式10份(正本1份、影本9份，請自行影印)，連同候選人相關資料1份(按頒授要點第9項第1點規定依序檢齊，免備函)，一併掛號寄送：臺北市中正區忠孝東路1段140號11樓中華民國志願服務協會。 |
| 四、 | 推薦表及影印文件一律請用A4規格紙張雙面影印，以期文件整齊(所送資料概不退還，請自行留底)；另為資料整理方便，所送推薦表一式10份及相關文件1份請依序檢齊用長尾夾夾牢即可，切勿使用資料簿或資料(透明)夾。 |
| 五、 | 有關本獎勵推薦文件涉及候選人個人資料部分，務必請各推薦單位徵得候選人本人同意後，方得予以推薦。 |
| 六、 | 頒授要點及相關空白表件電子檔請至該會網站(http://www.vol.org.tw/)-「資源分享」下載，上開資訊同步於本市志願服務整合資訊平台(https://vsty.tycg.gov.tw/)公告周知。 |

| 八、 | 檢附活動頒授要點及推薦表等各1份。 |
