| 一、 | 依據運動部115年5月6日運適(一)字第1150012806號函辦理。 |
| 二、 | 運動部委請國立臺灣體育運動大學辦理「115年適應運動調查計畫：身心障礙者運動參與現況與需求分析」（下稱本計畫），以建構我國適應運動推動所需之基礎統計資料，落實對身心障礙者運動平權之政策責任。 |
| 三、 | 旨揭計畫結合115年全國身心障礙國民運動會（簡稱115全障運），期蒐集身心障礙競技選手在訓練參賽過程之需求，以及賽會支援者對適應運動推動現況之建議。 |
| 四、 | 為不影響賽事進行，並兼顧不同障別選手及相關人員填答之便利性，本問卷調查將採線上方式進行（填答網址：https://www.surveycake.com/s/vAQW8），現場亦會安排訪員提供紙本問卷並進行適當協助；聯誼性活動（特奧）之參與者，將另行規劃於比賽前後採團體施測。本次調查作業另提供禮券供填答者抽獎。 |
