| 一、 | 依據雲林縣政府115年3月9日府教課二字第1152414671號函辦理。 |
| 二、 | 115年度教師專業成長研習實施計畫-夢的N次方素養工作坊-中一區雲林場研習資訊如下： |
| (一) | 研習時間：115年5月30日（星期六）上午9時起至下午5時、115年5月31日（星期日）上午9時起至下午4時止，計2日。 |
| (二) | 研習地點：雲林縣立雲林國民中學（地址：雲林縣斗六市明德路599號）。 |
| (三) | 開設課程：由夢的N次方團隊講師及雲林縣教師共同規劃課程，開設20班，每班課程12小時，預計錄取學員629人。 |
| (四) | 報名時間： |
| １、 | 第一階段報名時間：115年3月9日（星期一）上午8時起至115年3月29日（星期日）下午12時止，於115年4月1日（星期三）下午6時前公告錄取名單，若報名期間該班次已逾額滿上限，得提前結束該班別之報名作業。 |
| ２、 | 第二階段報名時間（僅開放尚未額滿班次）：115年4月12日（星期四）上午9時起至115年4月20日（星期日）24時止，並於115年4月13日（星期一）下午5時前公告錄取名單。 |
| (五) | 報名方式： |
| １、 | 第一階段：請至「全國教師在職進修網」，查詢課程代碼，點研習名稱進入報名。 |
| ２、 | 第二階段：請至「夢的N次方」網站（網址：https://sites.google.com/view/dream-practicer/自主/研習資訊?authuser=0），點選雲林場，於第二階段報名處點選網址報名。 |
| (六) | 本案研習以雲林縣現職教師優先參加，其他縣市教師、代理代課教師、師資培育生亦歡迎報名，錄取順序請參閱計畫。 |
| (七) | 本案研習報名人員請於公告日，自行查詢錄取名單；經錄取人員須全程參加研習，研習時數以學員實際簽到退紀錄核實核發，未確實簽到退者不得補簽，並視同自行放棄研習時數。 |
| (八) | 學員注意事項： |
| １、 | 開放取消報名及遞補作業：即日起至115年5月8日（星期五）止，逾期不候。 |
| ２、 | 取消報名方式：請統一致電夢N辦公室，電話(04)2218-3116或電子信箱dream.k12ea@gmail.com。 |
| ３、 | 備取通知方式：採E-mail方式通知，錄取通知信將寄送至報名時所填寫之信箱，不再另行公告。 |
| ４、 | 因活動場地周邊停車位有限，請盡量利用大眾運輸工具，雲林國中校內亦不提供學員停車位。 |
| ５、 | 為落實數位協力核心目標，請參與學員自備數位學習載具。 |
| ６、 | 為響應環保，減少垃圾量，請自備餐具及水杯。 |
| ７、 | 本研習不提供學員住宿。 |
| 三、 | 檢附實施計畫及各場次課程代碼。 |
