| 主旨： | 有關本校辦理115年度區域性資優教育充實方案：「視」機而動──視覺暫留創意應用與實作，請惠予公告周知並鼓勵貴校114學年度三、四年級學生踴躍報名參加，請查照。 |
| 說明： |
| 一、 | 依據桃園市西門國小「115年度區域性多元資優教育充實方案實施計畫」辦理。 |
| 二、 | 旨揭活動訊息如下： |
| (一) | 活動時間：115年7月1日（三）至7月3日（五），每日上午9時至下午4時，共3天。 |
| (二) | 活動地點：桃園市桃園區西門國小（桃園市桃園區莒光街15號）。 |
| (三) | 報名資格及人數：本市114學年度三、四年級學生，共計30名。 |
| １、 | 順位一：114學年度就讀本市國小三、四年級且通過本市資優鑑定之一般智能或創造力資賦優異學生。 |
| ２、 | 順位二：114學年度就讀本市國小三、四年級學生具創造才能特質，經專家學者、指導教師或家長觀察推薦者。 |
| (四) | 報名時間：即日起至115年6月12日（五），紙本檔及電子檔寄出，並收到電子郵件回覆者，即報名成功。 |
| (五) | 報名方式：採學校團體推薦報名，每校至多推薦五名，另備取兩名。 |
| (六) | 收件方式： |
| １、 | 各校將核章後「個人報名表暨家長同意書」（附件一）、「團體報名表」（附件二）、「區域方案課前任務單」（附件三），及非資優資格學生之創造力觀察推薦檢核表（附件四）紙本寄至西門國小輔導室（330桃園市桃園區莒光街15號）。 |
| ２、 | 請將「團體報名表」電子檔之word檔及其他報名資料之PDF檔（檔名: 00學校—115年度資優方案），寄至電子郵件(simes612@simes.tyc.edu.tw)輔導室特教組黃祐璿組長收。 |
| (七) | 錄取標準： |
| １、 | 依上列報名資格之順位錄取，並參考學生資優特質檢核表（附件四）填寫內容，具備創造力才能特質，且敘述中呈現對人文科學有強烈學習動機、善於觀察、具有解決問題能力等特質者，優先錄取。 |
| ２、 | 每校最多推薦五名學生，另備取二名，遇有缺額，由備取名單中遞補，共錄取30名，額滿為止。 |
| ３、 | 錄取名單於115年6月22日（星期一）16:00後公告於本校網頁 https://www.simes.tyc.edu.tw/nss/p/index。 |
| (八) | 報名費用：參加學生每人酌收 500元，以支應相關費用（午餐費等），多退少補，於活動開始第一天繳交，若活動開始，則不因個人請假原因再另行退費。 |
