| 主旨： | 函轉本府衛生局辦理115年桃園市「健康食動力-體位FUN 輕鬆 桃園學子向前衝2.0」活動簡章及活動海報各1份，請貴學校鼓勵學生踴躍參與，請查照。 |
| 說明： |
| 一、 | 依據本府衛生局115年3月30日桃衛健字第1150027764號函辦理。 |
| 二、 | 為提升本市7至17歲兒童及青少年健康體位知能，該局規劃「主題遊戲抽獎趣」、「任務挑戰抽好禮」、「學校獎勵機制」及「教師獎勵機制」 等線上活動，希冀透過學習正確飲食方式及運動習慣，同時將健康知識融入家庭生活落實健康體位，特辦理旨揭活動。 |
| 三、 | 活動資訊如下： |
| (一) | 活動網址：https://reurl.cc/N2Zyjx。 |
| (二) | 活動時間：115年4月1日至115年6月30日。 |
| (三) | 活動對象：本市國中小、高中職、五專1至3年級學生(含應屆) |
| (四) | 活動主題及抽獎辦法：參與「主題遊戲抽獎趣」、「任務挑戰抽好禮」完成任一項主題活動即可獲得1次抽獎機會。 |
| (五) | 獎勵機制： |
| １、 | 「學生個人獎」：頭獎5000元禮券1名、貳獎3000元禮券15名、參獎1000元禮券25名、運動水壺250名、精美帆布袋餐具組250名。 |
| ２、 | 「學校推廣獎」：各行政區學校參與率第一名，提供4,000元獎勵禮券，共計13所學校。 |
| ３、 | 「老師推廣獎」：協助學生完成活動達30人次以上之學校教職員，符合資格並通過審核者，抽出500元禮券20名（或等值獎勵品）。 |
| (六) | 活動規範： |
| １、 | 得獎公告：於活動結束後20日內(不含例假日)將符合抽獎資格之資料，以電腦系統隨機亂數產出得獎名單。並於8月10日公告於活動網站及該局臉書粉絲專頁，不另行通知或取得參加者同意。 |
| ２、 | 領獎辦法： |
| (１) | 依得獎者填寫之學校資訊寄發獎品並索取領據，得獎者應於期限內提供相關得獎者資訊，以利禮券(品)寄送。 |
| (２) | 得獎者為在校生者則統一送達就讀學所，非在校生者則依所提供資訊送達，請務必填寫正確資訊。 |
