| 主旨： | 有關本校辦理桃園市114學年度教育部國民及學前教育署戶外教育與海洋教育計畫-學校推展優質戶外教育路線『綠色任務GO~走進楓樹的家』活動，歡迎各校踴躍報名參加。 |
| 說明： | 經詢問承辦方，回覆各校報名踴躍，各梯次名額已滿。 |
| 一、 | 依據114年9月15日桃教小字第第11400869901號函辦理。 |
| 二、 | 活動期程:114年12月至115年1月合計辦理11梯次，各場次日期請參閱附件五。 |
| 三、 | 活動說明及報名方式如下： |
| (一) | 活動內容路線及課程說明如附件二，本活動預計辦理11所學校，每校至多得申請1梯次，一梯次不超過30人，活動全程免費。 |
| (二) | 報名方式：線上報名(https://forms.gle/VQM8eU1JhBcEtCTD8)至114年11月21日(星期五)截止，依報名先後順序，額滿即關閉表單，核定學校名單將另行通知。 |
| 四、 | 本活動每校補助14,650元，內容如下: |
| (一) | 交通費每校補助1車10,000元。 |
| (二) | 膳費及茶水費每生補助120元(不含教師，膳費100元及茶水費20元，每校30生，計3,600元) 。 |
| (三) | 保險費每生補助35元(不含教師，每校30生，計1,050元) |
| 五、 | 成果及經費核銷:請於活動結束後二週內，每校補助款之支出原始憑證正本(黏貼憑證皆需各校校內核章完妥)，檢據送回楓樹國小學務處彙整並辦理經費撥付。檢據內容如下： |
| (一) | 統一收據(依實際金額開立，繳款人：桃園市龜山區楓樹國民小學，註明學校帳戶戶名及統編) |
| (二) | 紙本學習成果、戶外教育學習回饋單二份及活動名冊簽到表(附件三、四)，成果及回饋單掃描電子檔請寄至信箱: kuanjuihua@fsps.tyc.edu.tw，檔名:**國小戶外教育與海洋教育計畫成果。 |
| (三) | 收支結算表(附件五) |
| (四) | 活動支出之原始黏貼憑證正本（請務必自行影印副本留存，供貴校校內核銷。) |
| 六、 | 檢附課程活動流程及路線、活動成果、回饋單及學員名冊簽到表如附件，若有相關問題，請逕洽楓樹國小學務主任官瑞華(電話：03-3206166#310)。 |
