| 二、 | 旨揭開班相關資訊說明如下： |
| (一) | 開設班別：雙語體育班、雙語綜合班，共計開設2班，雙語體育班尚有19名缺額、雙語綜合班尚有16名缺額。 |
| (二) | 上課時間： |
| １、 | 雙語體育班：自113年8月12日(星期一)至8月16日(星期五)、8月19日(星期一)至8月22日(星期四) |
| ２、 | 雙語綜合班：自113年8月12日(星期一)至8月16日(星期五)、8月19日(星期一)至8月22日(星期四) |
| (三) | 招生對象：國民小學具備國小合格教師證書之現職教師，達到CEFR架構之B1級(含)以上者優先錄取。 |
| 三、 | 報名資訊： |
| (一) | 報名時間：即日起至113年6月13日(星期四)下午5時止 |
| (二) | 報名方式：線上報名與紙本報名。 |
| １、 | 紙本報名：請於113年6月13日(星期四)前以掛號郵寄至「10671 臺北市大安區和平東路2段134號」，收件人欄位請填寫「國立臺北教育大學師資培育處雙語教學研究中心(A700b室)收」(時間以郵戳為憑)。 |
| ２、 | 線上報名：請於113年6月13日(星期四)前卓參附件之實施計畫書、報名表件與相關附件，並於本表單填寫您的報名資訊，同時將您的報名文件[含下列(1)至(4)項)]副本掃描PDF檔合併後上傳至後方報名連結，檔名請統一為:『您的全名_服務學校_在職雙語教師增能學分班報名』。線上報名連結：https://forms.gle/9ftaPNAocPCirhXY6 |
| (三) | 報名所需資料如下(詳如簡章)： |
| １、 | 紙本報名表正本(需加蓋學校印信)。 |
| ２、 | 當學年度在職證明書或聘書影本。 |
| ３、 | 國民小學合格教師證書影本(影本須註記與正本相符字樣並親簽)。 |
| ４、 | CEFR B1等級之英語能力分級測驗通過證明，或CEFR B2等級或以上之英語能力分級測驗（聽、說、讀、寫）通過證明(有則檢附、無則免附)。 |
| ５、 | 確認已完成線上表單報名四、檢附報名表與學分班執行計畫簡章、報名表件、課表。 |
