| 一、 | 依據國立臺灣師範大學113年5月16日師大數學中字第1131013927號函辦理。 |
| 二、 | 旨案相關資訊臚列如下： |
| (一) | 申請資格： |
| １、 | 授課教師需取得該中心所培訓相關課程之證書。 |
| ２、 | 申請學校可另覓領有相關證書的教師參與數學營授課，跨校合作。 |
| ３、 | 尚未完成培訓者可提出培訓構想計畫且必須於報名截止前完成培訓並取得證書。 |
| (二) | 申請期程：113年5月13日(星期一）至113年6月14日（星期五）下午5 時（因經費有限，將視報名狀況提早結束或延長報名時間）。 |
| (三) | 申請方式： |
| １、 | 一律採線上報名，報名網址：https://teach.cloud.ncnu.edu.tw/（教育部國民及學前教育署國中小課程教學計畫填報系統）。 |
| ２、 | 請教師由教育雲端帳號登入註冊後點選「好好玩數學研習營計畫」，並依國小中年 級（三、四年級）、國小高年級（五、六年級）或國中組分別提出申請並填寫資料。 |
| ３、 | 資料填寫完畢後印出計畫書，並經申請人及校長簽章後，於 113年6月14日（星期五）下午5時前掃描上傳至填報系統並點選提交即為完成申請，逾期不予受理。 |
| (四) | 相關申請經審查後，預計113年6月24日（星期一）於該中心網站公告核定通過名單（將視審查期程調整公告日期）。 |
| (五) | 活動規範： |
| １、 | 辦理期程：113年7月1日（星期一）至113年8月29日（星期四）。 |
| ２、 | 授課內容：依取得該中心培訓證書所對應之相關數學奠基模組課程（模組教案見中心網站下載專區：https://www.ime.ntnu.edu.tw/index.php/product-ground/）。 |
| ３、 | 申請組別：國小中年級組、國小高年級組或國中組（請分別提出申請）。 |
| ４、 | 申請班次：1個模組為1班次，每班至少辦理2班次（需為2的倍數），且每校至多申請 24 班次。 |
| (六) | 相關資訊亦公告於該中心網站（https://www.ime.ntnu.edu.tw/）。 |
